A variety of specialized health management programs may be available to you and your family members. While some programs are available to all Citi employees and their family members, others require enrollment in a Citi medical plan.

Cancer Support
Dealing with a serious illness like cancer is an overwhelming experience. Citi offers a variety of comprehensive support options to help you and your family members cope with a cancer diagnosis and navigate the treatment process.
If you’re enrolled in an Aetna or Anthem medical plan, you have access to specialized programs through the plan, as described below. In addition, all Citi employees have access to MSK Direct. If you’re eligible for both MSK Direct and a medical plan program, you can reach out to either option to get started. You’ll be referred as appropriate to ensure that you have access to all the support options available to you.
MSK Direct (for Everyone)
We’ve partnered with Memorial Sloan Kettering Cancer Center (MSK) to provide a program that connects you and your family with expert care from cancer specialists at MSK while providing logistical and emotional support along the way.
All Citi employees and their family members have access to this program, regardless of enrollment in other health benefits offered by Citi. There are no extra fees for most services — patients are responsible for the standard copays, coinsurance and deductibles for the medical services received from MSK, as would be the case with any other medical services received under Citi's medical plans. MSK Direct is considered an in-network facility for Citi’s Aetna and Anthem medical plans.*
MSK Direct can help by providing the following services:
- Scheduling an initial consult at MSK, usually within two business days, and exploring options for treatment at MSK
- Helping gather necessary medical records for the first MSK appointment
- Meeting patients at their initial appointment at MSK to provide support, logistical assistance and introductions to their clinical team
- Facilitating remote second opinions for patients who are unable to travel to MSK (fees apply)
- Recommending a local facility if the patient lives far from MSK and prefers to be treated closer to home
- Providing a second opinion on a cancer diagnosis and/or care plan that are from another facility (fees apply; note that you may request an expert opinion through 2nd.MD at no cost to you)
To get started with MSK Direct, call the dedicated phone line for Citi employees and their family members at 1 (833) 825-4558 between 8:30 a.m. and 5:30 p.m. ET, Monday through Friday. Calls outside of these hours will be returned the next business day. An employee’s family members will need to state their relationship to the Citi employee.

Aetna Cancer Support Center
If you’re enrolled in an Aetna medical plan, you and your covered family members have access to the Aetna Cancer Support Center. This program is designed to make a difficult time a little easier by providing helpful online resources and personalized support from a dedicated advocate.
The Cancer Support Center can help by providing:
- Complete online resources for your treatment journey, including checklists and educational videos
- One-on-one support from a knowledgeable, compassionate advocate, as well as help with coordinating logistics
- Explanation of your medical coverage and answers to your financial questions
- Assistance with genetic testing and counseling to guide your or your family’s care plan

Anthem Concierge Cancer Care
If you’re enrolled in an Anthem medical plan, you and your covered family members have access to the Concierge Cancer Care program. This program provides personalized support every step of the way, so you can focus more on your health and let the experts at Anthem assist you through the treatment process.
The Concierge Cancer Care program can help by:
- Finding the most appropriate care by matching you with hospitals that specialize in the care you need
- Coordinating logistics, including reviewing treatment options, scheduling appointments and identifying next steps
- Expediting the clinical trial authorization process and helping to book your travel and lodging for clinical trials, as applicable
- Addressing financial questions, clarifying coverage and confirming pre-authorizations
- Offering emotional support throughout your course of treatment
- Arranging a second opinion pertaining to your initial diagnosis at no cost to you to ensure that the appropriate treatment is received

Ways to Reduce Risk with Citi’s Help
In addition to using these resources and guidance from your doctor, Citi supports you with other proven ways to reduce risk.
Regular Screenings Based on Age, Gender and Family History
Did you know that screenings for breast, cervical, prostate, colon and skin cancer are covered at no cost to you when you use an in-network provider through Citi’s medical plan? Certain screenings are recommended based on your age, gender and/or family history. For example, current guidelines for men recommend starting discussions with your doctor about screening for prostate cancer in your 40s. For women, current guidelines recommend starting discussions with your doctor about cervical cancer testing (such as pap tests) at age 25. Once you reach age 40, you may talk to your doctor about receiving an annual mammogram. Colon cancer screenings are recommended once you reach age 45 (or earlier if you have a family history of colon cancer or colorectal polyps).
Healthy Behaviors
Healthy behaviors may include reducing alcohol consumption, quitting smoking if applicable, and consuming fewer highly processed foods. The Live Well at Citi Program offers a variety of no-cost resources, including coaching to support you in making healthy habits and sticking to them.
Chronic Condition Support
Living with a chronic condition is challenging. Citi’s benefits offer specialized resources to help you manage your situation and live your best life. If you are enrolled in a Citi medical plan through Aetna or Anthem, you may be contacted by a medical professional from your plan carrier to participate in a specific program to address your chronic health conditions. Note: If you’re enrolled in an HMO through Citi, you generally aren’t eligible for the Live Well Chronic Condition Management Program or the Live Well Reward.
Chronic Condition Management Programs
Citi offers Chronic Condition Management Programs through your medical carriers, Aetna and Anthem. Both carriers support a variety of different conditions, including asthma, diabetes, heart disease, cancer, low back pain and digestive conditions. The programs help you:
- Manage your condition
- Lower your risks for new conditions
- Work better with your doctor
- Take your medicine safely
- Find helpful online and community resources
Depending on your situation, you may receive a call or letter from Aetna or Anthem. If you enroll in the Chronic Condition Management Program upon invitation, you will work directly with an Aetna or Anthem provider.
Diabetes Management Program
If you or a family member has diabetes and are enrolled in a Citi medical plan, you can participate in the Diabetes Management Program through Transform Diabetes Care (TDC). There is no additional cost for this program.

Live Well — and Earn Rewards!
Completing a Chronic Condition Management Program is one of the healthy activities you and your eligible spouse/partner can do to earn rewards, which you can redeem for gift cards or use to make charitable donations on the Personify Health platform. You must be invited by a nurse from your plan to participate in a Chronic Condition Management Program, through Aetna or Anthem, and complete three coaching sessions between January 1 and September 30 to earn points that accumulate toward your rewards. Eating Well and Exercising Regularly
The Live Well at Citi Program can connect you with free resources supporting healthy nutrition and exercise programs, all at no cost to you. Plus, through Personify Health, you may be able to earn up to $280 in rewards for healthy behaviors like exercising, losing weight or quitting tobacco. Citi also offers Wellhub so you can tailor your exercise experience to your goals and budget — with access to 50,000+ gyms, studios and wellness apps.
Routine Health Screenings
Routine checkups may include a biometric screening, which lets you and your doctor see how you’re doing with your cholesterol, blood sugar and blood pressure — key indicators of your cardiovascular health. Preventive care with an in-network provider is free through Citi’s medical plans. You can also earn points through the Live Well at Citi Program when you receive certain preventive care services.
Diabetes Management
Transform Diabetes Care (TDC) is a comprehensive diabetes care and management program provided through CVS Health. Based on your needs, TDC helps you take control of your health, at home or on the go, with easy-to-use technology and direct access to registered nurses, coaches and dieticians who provide support and resources customized to your needs.
Citi offers TDC at no cost to employees and family members enrolled in a Citi medical plan.
How Can TDC Help You?
TDC’s customized, proactive approach is based on your needs and encourages more frequent blood glucose checks, provides just-in-time outreach from diabetes experts and automates supply ordering and blood glucose tracking. Most importantly, you’ll receive personalized, real-time support and education that can enable more confident self-management and improved glycemic control.

TDC provides timely, effective, individualized support through your choice of face-to-face, phone or digital communication from pharmacists, health coaches and Certified Diabetes Care and Education Specialists (CDCES), all based on your specific needs within five key areas:

- Self-monitoring blood glucose (SMBG) testing with digital app support
- Cellular SMBG testing with support from a Certified Diabetes Care Nurse
Lifestyle and Complication Management
- Tobacco cessation

- Support with things that may interfere with your health, such as affording nutritious food and prescribed medications

*MinuteClinic® services are free to Citi employees and family members enrolled in the Choice Plan, In-network Only Plan or High Deductible Plan with HSA. In the High Deductible Plan with HSA, your plan deductible will apply, then you'll pay nothing after meeting the deductible. Note that your Citi medical plan covers preventive care in full, with no cost to you, when received from any in-network health care provider.

Good Nutrition
Forming conducive lifestyle habits around nutrition, such as getting more fiber in your diet, can be challenging for some. With the Live Well at Citi Program, you can get guidance from coaches at no cost to you.
You may also find an in-network registered dietitian near you through your Citi medical plan.
Regular Exercise
It’s been shown that regular exercise can help support overall metabolic health, such as by helping to lower blood sugar and increasing your sensitivity to insulin. Regular exercise may also support weight loss and management.
Citi has onsite fitness centers at certain locations and options for gym memberships through Wellhub, so you can tailor the options to your needs and budget.
