You may be contacted by a medical professional from your medical plan to participate in a specific program to address your chronic health conditions. Note: If you’re enrolled in an HMO through Citi, you generally aren’t eligible for the Live Well Chronic Condition Management Program or the Live Well Reward.

Chronic Condition Management Programs

Citi offers Chronic Condition Management Programs through your medical carriers, Aetna and Anthem. Both carriers support a variety of different conditions, including asthma, diabetes, heart disease, cancer, low back pain and digestive conditions. The programs help you:

  • Find helpful online and community resources

Why do I have to be invited to participate?

The Live Well Chronic Condition Management Programs are designed to help participants manage certain chronic health conditions. Therefore, you must be identified (e.g., through your medical claims) as having a chronic health condition in order to participate.

When will my Live Well Rewards be available to me after I complete my Live Well Chronic Condition Management Program?

You’ll receive Live Well Rewards for completing the Live Well Chronic Condition Management Program within approximately one month after completing the program.

If I have participated in the in the Live Well Chronic Condition Management Program in the past but have additional chronic conditions, can I still be eligible to earn Live Well Rewards for the current year?

Yes, if you’re invited to participate. Your medical plan carrier will determine which chronic conditions can still be addressed through the program, and you’ll be notified accordingly. Note: This program and Live Well Rewards aren’t available to you if you’re enrolled in a Citi HMO.

What happens if I complete the Live Well Chronic Condition Management Program and then leave Citi for any reason? Will I still receive Live Well Rewards?

If you’ve completed your program and your Live Well Rewards has been posted, you can redeem them for a gift card as long as you’re employed by Citi when you submit your gift card order. If you terminate your employment with Citi prior to redeeming your Live Well Rewards, you will forfeit any Rewards earned at the time of your termination.
