If you have a chronic condition, Citi provides you with the resources to live well. You may be contacted by a medical professional from your medical plan to participate in a specific program to address your chronic health conditions. Note: If you’re enrolled in an HMO through Citi, you generally aren’t eligible for the Live Well Chronic Condition Management Program or the Live Well Reward.

iconChronic Condition Management Programs

Citi offers Chronic Condition Management Programs through your medical carriers, Aetna and Anthem. Both carriers support a variety of different conditions, including asthma, diabetes, heart disease, cancer, low back pain and digestive conditions. The programs help you:

  • Manage your condition
  • Lower your risks for new conditions
  • Work better with your doctor
  • Take your medicine safely
  • Find helpful online and community resources

 

Depending on your situation, you may receive a call or letter from Aetna or Anthem. If you enroll in the Chronic Condition Management Program upon invitation, you will work directly with an Aetna or Anthem provider. For more information, visit the Benefits Handbook.

Live Well – and Earn Rewards!

Completing a Chronic Condition Management Program is one of the Healthy Activities you can do to earn Live Well Rewards, which you can redeem for gift cards. You must be invited by a nurse from your plan to participate in a Chronic Condition Management Program, through Aetna or Anthem, and complete three coaching sessions between January 1, 2020, and September 30, 2020, to earn $100 in Live Well Rewards.

Completing a Chronic Condition Management Program is one of the Healthy Activities you can do to earn Live Well Rewards, which you can redeem for gift cards. If you’re invited to participate in a Chronic Condition Management Program and you complete three coaching sessions between January 1, 2020, and September 30, 2020, you can earn $100 in Live Well Rewards. This $100 counts toward your $450 annual maximum Live Well Rewards.

This program is available to employees and spouses/partners enrolled in a Citi medical plan through Aetna or Anthem. If you’re enrolled in one of these plans, a nurse from your plan may reach out to invite you to participate in order to help manage your condition. You must be invited to participate in the program. If you’re enrolled in an HMO through Citi, you generally aren’t eligible for the Live Well Chronic Condition Management Program or the Live Well Reward.

Yes. New employees who enroll in one of the Aetna or Anthem medical plans are eligible. If you’re enrolled in one of these plans, you must be invited to participate in order to earn a Live Well Reward.

iconWhy do I have to be invited to participate?

The Live Well Chronic Condition Management Programs are designed to help participants manage certain chronic health conditions. Therefore, you must be identified (e.g., through your medical claims) as having a chronic health condition in order to participate.

iconWhen will my Live Well Reward be available to me after I complete my Live Well Chronic Condition Management Program?

You’ll receive the Live Well Reward for completing the Live Well Chronic Condition Management Program approximately 30 days from the end of the quarter in which you complete the program. For example, if you complete the program on May 20, you’ll receive your credit in the form of a gift card before July 1.

iconWhat happens if I participated in the Live Well Chronic Condition Management Program last year and received a $300 Live Well Reward and have additional chronic conditions that I want to work on this year? Will I be eligible for additional coaching sessions and an additional $100 Live Well Reward for the current year?

Yes, if you’re invited to participate. Your medical plan carrier will determine which chronic conditions can still be addressed through the program, and you’ll be notified accordingly. Note: This program and Live Well Reward aren’t available to you if you’re enrolled in a Citi HMO.

iconWhat happens if I work with a nurse through my medical plan for three sessions and then leave Citi for any reason? Will I still receive the $100 Live Well Reward?

If you’ve completed your program and your Live Well Reward has been posted, you can redeem it for a gift card as long as you’re employed by Citi when you submit your gift card order. If you terminate your employment with Citi prior to redeeming your Live Well Rewards for a gift card, you will forfeit the $100 Live Well Reward, as well as other rewards for which you haven’t ordered a gift card at the time of your termination.
